Required Forms
The following forms are required to be completed by all TTUHSC School of Allied Health Sciences new students. Please print, sign and date all forms and return them to the SOAHS Office of Admissions and Student Affairs. Training
As a new TTUHSC student you are required to complete the following training sessions. Upon completion of each training session you will have the option to print the completion certificate. Please print each completion certificate, sign, date and mail the certificates to the following address:

You will need an eraider username and password to complete the training session.

- STEPS, (CLS Students only) (pdf)
You are required to complete the STEPS & Lab Safety Training sessions before you begin the CLS program. You must print the completion certificates at the end of the training session.
